Job summary An exciting opportunity has arisen for a dynamic and driven Physiotherapist to join the Acute Therapies DMOP Team at Northwick Park Hospital. You and your team will be responsible for delivering specialist Physiotherapy services within the DMOP team. You will be an essential part of the large multidisciplinary team and part of the wider inpatient therapy team. This well-established team covers four DMOP inpatients wards and Better at Home service, that aims to deliver timely post discharge rehabilitation in the community.The successful applicant will have significant experience at Band7 level and be able to demonstrate leadership qualities and management acumen. You will have an opportunity to work alongside clinical specialist developing skills in a wide range of conditions, working closely with MDT. You will be required to work in conjunction with the team lead to develop and support the rest of the team. Main duties of the job As an autonomous practitioner to assess and treat own case load and provide a highly specialist Care of the Elderly (DMOP) therapy service to patients. To work alongside the multi-disciplinary team and other external agencies to ensure highly specialist treatment plans. To work as a member of the multidisciplinary team (MDT), assessing and treating patients with highly complex physical/social and cognitive issues associated with DMOP patients and maintaining accurate and comprehensive patient records. To work closely with the DMOP MDT and cross-site clinical and therapy leads to support the development of the DMOP therapy service. To work collaboratively with patients, carers and with colleagues in health and social services to ensure effective service planning, policy and delivery of services to their specific patient group. To contribute to the management of all aspects of the DMOP therapy Service. This includes prioritizing and organizing all referrals to the DMOP inpatients and allocation of patients.
