An excellent opportunity has arisen at Whitburn Surgery for an Advanced Nurse Practitioner to join our friendly practice to help us deliver high quality care.
Main duties of the job
* We are looking for either a FT 37.5hrs or 2 x PT 16-24 hours per week.
* The postholder will need to triage the needs of the presenting patient from taking medical history, assessment, diagnosis, and treatment ensuring any necessary referrals are made whilst acting within their professional boundaries.
* The post holder must be a registered general nurse (currently registered with the Nursing and Midwifery Council), have a recognised NP qualification (minimum of degree level or equivalent) and also be an independent Nurse Prescriber.

The Surgery is operated by South Tyneside Health Collaboration (STHC) which is a GP Federation that provides primary care services at scale throughout the Borough.
Our Practice consists of 3 Salaried GPs, 2 Practice Nurses, 2 GPAs, Practice Manager and an administration team of 8.
Job responsibilities
The following are the core responsibilities of the Advanced Clinical Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
* Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice.
* Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of their own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.

* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary or specialist care as required.
* Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively.
* Develop, implement and embed an effective call/recall system.
* Critically appraise and synthesize the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others.
* Develop and implement robust governance systems and contribute to practice targets QOF, etc., complying with local and regional guidance.
* Chaperone patients where necessary.
* Support the team in dealing with clinical emergencies.
* Support the team with all safeguarding matters in accordance with local and national policies.
* Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
* Deliver opportunistic health promotion where appropriate.
* Participate in enhanced access.
* Work in other STHC practices if the need arises.
Person Specification
Qualifications
* Educated to Masters degree as detailed within the HEE document titled - Multi-professional framework for advanced clinical practice in England.
* Qualified independent prescriber - V300.
* Registered on their appropriate professional register.
* Teaching qualification.
* ALS and PALS.

* Clinical knowledge and skills appropriate to advanced clinical practitioner.
* Understand the importance of evidence-based practice.
* Broad knowledge of clinical governance.
* Ability to record accurate clinical notes.
* Ability to work within own scope of practice and understand when to refer to GPs.
