Job summary We have an exciting opportunity for an Associate Community Matron for Parkinson's Disease to join our team on the Isle of Wight. As a Parkinson's Nurse Specialist you will provide information, advice and specialist support for people with Parkinson's and for their carers and families as well as healthcare professionals involved in their care. Main duties of the job The purpose of the Community Matron for Parkinson's disease is primarily to maintain and improve quality of life for people with Parkinson's disease, provide support and education to people with Parkinson's Disease and their families in any location, and support and provide a seamless service from diagnosis to palliative care. The Parkinson's disease community matron will act as an expert resource, providing advice, support and education for professional staff in the primary, secondary, voluntary and private care sectors. This will encompass the nursing care and management of people with Parkinson's disease (PD), working closely with patients, carers, consultants and general practitioners, therapists, social services and the voluntary sector advising on complex nursing care issues. Highly specialist and comprehensive assessment of individual patient's needs, will lead to the setting and implementing of programmes of care for patients, linking with primary care and hospital staff to ensure care is properly co-ordinated.
