Work with GPs to support delivery and development of the Womens Health Hub. Carry out LD annual health checks, ongoing reviews and follow ups. Accurately perform and record clinical observations, including BP, knowledge of when to escalate. Carry out long term condition reviews and any necessary follow ups. Work with registered nurses delivering on key priorities areas across the PCN. Delivering high quality, compassionate care. Communicate risk issues as they arise to ensure safe care. Promoting health and independence for patients. Signposting to services such as menopause clinics. Qualified to deliver cervical screening, or willingness to train. Delivering holistic care with a whole person approach. Contributing to MDT and onward discussions. Working across the GP practices within the South Sefton PCN. Support new staff/HCAs IT competent, ability to accurately input data and produce reports using clinical systems. Understanding of clinical/information governance confidentiality and data protection. This role may vary dependent on the key priorities of the PCN. This may change in line with NHS England and changes in the key priorities within South Sefton PCN. Nurse Associate Qualification is essential. You will need this otherwise you will not be able to fulfill the role.
