Job summary The post holder will be employed by North Middlesex University Hospital (Enfield Community) with primary responsibility at Chase Farm Hospital on Cape Town and Canterbury wards. Occasionally the post holder may be expected to work across the wider team at Magnolia Unit and Virtual ward. Patients will be identified as being medically optimised but requires community support while waiting for either placement or package of care or requiring rehabilitation. Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing, patients on the ward and act as a clinical leader and role model to staff within the service, offering clinical advice on the care of patients with complex needs as required. You will work with the GP, colleagues and managers to support the integration of care pathways and patient care across community settings. Main duties of the job You will be expected to have skills in advanced physical assessment, diagnostic reasoning, nurse independent prescribing and managing patient with complex needs. Using these skills will assist in the recognition of early symptoms of disease exacerbation, acute illness and injuries. The post holder will co-ordinate care and navigate the patient through pathways across the primary, secondary care interface, including the prevention of unnecessary readmissions, facilitating appropriate ones through in-reach case finding from medical /surgical wards at NMUH and key partners North London (Barnet, Whittington, Royal Free hospital, UCLH for rehab patients for rehabilitation.
