Role Responsibility Kent County Councils Adult Short Stay Services currently has an exciting opportunity for a Pre-Admission Assessor and Team Leader to join our team at Blackburn Day Centre in Sheerness to cover long term leave. We are looking for a motivated and professional person who is committed to working in integrated social care teams, ready to embrace change and new ways of working. Our social care interventions are structured around the three pillars of excellent practice, exceptional innovation, and meaningful measures. Therefore, our work is orientated around building relationships with the individual, their families and carers. As a key part of KCC Adult Social Services core values, Adult Short Stay Services aim to provide enablement and support services to help people to remain living in their own home and achieve the outcomes that are important to them. You will be responsible for leading and managing the day centre, whilst complying with KCC policies and values. You will also work collaboratively with commissioners, partner organisations and the NHS to support pre-admission assessments for people being admitted to wider Adult Short Stay services. You will have a NVQ 3 in Health and Social Care, a NVQ 3 in Supervisory Management or equivalent and the ability/willingness to work towards level 5 in Health and Social Care or Management. You will have experience in working with older people, preferably in reablement/intermediate care and day care services. Due to the short term nature of this post, you will require minimal induction to the post- experience of running day centre services is therefore essential.
