Responsibilities
Conduct comprehensive assessments to identify the needs of clients and their families.
Develop, implement, and review care plans ensuring the protection and welfare of service users.
Provide ongoing support and guidance to individuals facing complex social challenges and mental health issues.
Collaborate with multi-disciplinary teams to deliver cohesive care and access to resources.
Mentor and supervise junior social workers and students, fostering their professional growth and skills development.
Engage with community organizations and stakeholders to facilitate access to local services and resources.
Maintain accurate and detailed documentation of case notes, assessments, and service plans in compliance with legal and organizational standards.
Requirements
A recognized degree in Social Work and current registration with the relevant social work regulatory body.
Significant experience in adult social work, including working with vulnerable populations and complex cases.
Demonstrated knowledge of relevant legislation, policies, and best practices in social work and adult care.
Strong assessment and analytical skills to evaluate clients' needs and plan appropriate interventions.
Excellent communication and interpersonal skills to build trust and rapport with clients and professionals alike.
Ability to work both independently and as part of a team, demonstrating leadership qualities and initiative.
Commitment to continuous professional development and ability to adapt to changing circumstances in social care.
