You will ensure that patient referrals are appropriately received, triaged, actioned, and reviewed and outcomes communicated with relevant parties as well as coordinating the planned work of the Neighbourhood Night Service. You will be a key point of contact for multi-disciplinary teams ensuring, communication is effective with staff on duty overnight and any other relevant parties.You will cover three shift patterns, ensuring the Neighbourhood Night Service operates over the 24 hour period and that there are handover opportunities between the night and day elements of the team. The three shifts are 09.00am-17.00pm, 14:00pm-22:00pm and 09.00-22.00pm over seven days per week and are allocated on a rota basis. You will also visit patients as part of your hours through the day to help support night assessment outcomes.You will work clinically/Triage during your shift times between 17:00-22:00pm 1-2 times per month, to maintain clinical skills and understand and support how the service operates.
