Main duties of the job
The post holder is required to work within a Multidisciplinary Team - with adults and children with a learning disability and associated complex health needs.
The post holder will be responsible for taking on appropriate assessment of a person's needs and undertaking interventions that aim to improve their quality of life and achieve positive outcomes. This will include planning how to do this alongside other professionals within the team. The post holder will provide education, knowledge, guidance, and support to student nurses and unqualified staff in the team and provide support to clients and their families.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications
* RNLD
Experience
* Experience in learning disability services
* Broad range of experience working in health & social care settings
Knowledge
* Demonstrate a good understanding of Learning Disability, including Mental Health, Forensics, Challenging Behaviour, and PLD/Complex Health. Awareness of current developments within a clinical setting.
* Awareness of current developments within Learning Disability services.
Personal Qualities
* Ability to communicate clearly and accurately
* Ability to adapt to new and changing circumstances
* Self-motivated
* Ability to prioritise and work proactively
* Ability to manage a designated caseload without direct supervision
* Commitment to continuous improvements to clients' care

£30,420 to £37,030 a year, pro rata.
