Job summary Surrey Downs Health & Care The Rehabilitation Support Worker role will function as part of the new and exciting Recovery At Home team by providing care, support and rehabilitation within a community setting to provide a seamless service and to support discharges from hospital or prevent admission to hospital. The post holder will undertake a range of delegated tasks, following a care plan and will report to the Team Lead. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care. Wherever possible patients should be supported home for ongoing assessments where this is not possible alternative pathways are available. Staying in hospital for longer than necessary has a negative impact on outcomes and ensuring patients are given the chance to continue their lives at home is vital for their long-term wellbeing. home works across organisational boundaries and as an integral part of the integrated health and care team is responsible for providing rapid access, enhanced, short term health and care support for people who would otherwise need emergency hospital admission as well for as those medically stable to return home from an acute hospital but requiring enhanced support as one stage in their on-going care pathway. The home service spans the acute and community providing joined up, co-ordinated care to local citizens with health and social care needs. The Recovery Athome Team will form an integral and essential part of Discharge to Assess, providing timely care and support to facilitate discharge from hospital and onward assessment and identification of need.
