We are seeking a motivated and experienced practice nurse to join our friendly multi-disciplinary team, to support the provision of high-quality healthcare to our growing and diverse patient list. This is an exciting opportunity to join our forward thinking and innovative practice as we grow, transform, and modernise pathways of care. The post holder will provide high quality primary care nursing services to patients registered with the practice, including screening, assessment, treatment and advice, in order to promote and facilitate a healthy lifestyle and provide preventative health care. The post holder will be based in the practice but will work as part of the integrated primary/community health care team, liaising with general practitioners, and other professionals, and statutory and voluntary agencies across professional and organisation boundaries. About the Candidate Motivated to provide the highest standards of care, you will be keen to support new ways of delivering healthcare and committed to undertaking the professional development required to meet the needs of the practice and our patient population. Consideration will be given to candidates who do not have direct primary care experience who have transferable knowledge and skills from another role. A full package of training and support will be available. You will possess excellent communication skills and work collaboratively with the practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
