Job responsibilities To promote the key values of palliative care within the surgery. To team build with a cohesive vision, linking community and hospital staff in this vision to provide excellent palliative and supportive care. To act as an interface with the independent, NHS and voluntary sections. To visit care homes within the surgery visiting boundaries and in special cases (where discussed with GP partners) for those outside of surgery visit boundaries to ensure continuity of care in the last days. To act as a role model, providing clear visible leadership in service development and management and palliative supportive care. To be accountable and responsible for standards of care and to monitor and evaluate service and patient care outcomes, using this information to continually improve the service. To interpret national policy and ensure compliance to quality and performance standards. To be accountable for the achievement of aligned quality performance indicators for the service. To provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. To be able to demonstrate safe, clinical decision making and expert care for patients with a palliative or supportive need in the surgeries. To act as a key resource in the effective working relationships with other healthcare providers and professionals. To act as a clinical lead within the team for palliative and supportive care. To lead in the development of specialist palliative care education and accreditation as per Gold Standards Framework accreditation. To co-ordinate/influence research and development agenda for specialist palliative and supportive care within the PCN. To work within the NMC code of conduct principles and scope of professional practice.
