Clinical Triage Nursing Sister (Integrated Single Point of Access)

If you are an innovative and forward-thinking Nurse who is ready to take on an exciting Clinical Triage role, then this post is for you! An exciting opportunity has arisen for an experienced Band 6 nurse to join a multidisciplinary team working within our Integrated Single Point of Access (ISPA). Your role will be to triage patient referrals that come into Coventry Community Physical Health Services and ensure our patients get the right care from the right team at the right time.
As an experienced Clinician, you will apply your clinical knowledge and risk assessment skills to triage and clearly identify urgent and non-urgent patient referrals and allocate and arrange visits to our patients within the community setting. The role is predominantly completing telephone triage so it's essential you have excellent communication skills as you will regularly be liaising with patients, carers and other professionals. You will also be IT competent and confident as you will be working with technology and multiple patient information systems.
As a valued team member your development will be supported through a robust induction program, a values based appraisal, one to one meetings, regular in-service training and clinical and managerial supervision. You will be working as part of a multi professional team so there is opportunity for joint working and collaborative learning across a range of nurses and therapists.
Main duties of the job
Our working hours are between 8am-8pm 7 days a week and you will be required to work shifts within those times and days. Training opportunities are supported by our Practice Based Facilitators and is actively encouraged by Team Leads.
1. To have continuing responsibility to prioritise referrals through the integrated single point of access (ISPA) using sound clinical knowledge and risk assessment skills to ensure patients are transferred to the most appropriate service to meet their health care needs.
2. To maintain knowledge, skills and attitudes necessary to deliver an evidence-based service that is responsive to the changing needs of the population and to ensure all contacts with the ISPA service reach an effective and appropriate outcome for the patient.
3. To participate in peer to peer supervision, act as a senior resource and support with other team members.
4. To be responsible for the provision of effective learning experiences for pre- and post-registration students.
5. To effectively coordinate the resources available within Community Services to maximise access to services or sign post to other services available.

Job responsibilities
1. Undertake comprehensive holistic telephone or other virtual technology assessment of the patients presenting problem.
2. With consent, liaise with relevant healthcare or other professionals, relatives and carers to acquire necessary clinical information to inform comprehensive triage of referral.
3. Ensure timely escalation of present time demand and capacity issues in the team daily to clinical and operational leads.
4. Allocate referrals requiring an urgent (timed) response to the rapid response team to ensure timely patient care.
5. Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.
6. Involve patients and carers in planning and delivery of care.
7. To competently perform a range of clinical triage skills related to the post.
8. Ability to work without direct supervision and to solve problems.
9. Work on a rota basis to meet the service requirements.
10. Maintain a professional image.
11. Attend professional meetings and case conferences as deemed necessary.
