An exciting opportunity has arisento join the community nursing service in Halton. Are you an enthusiastic andmotivated Nursing Associate looking for a new challenge? You will join a teamof nurses delivering high quality care to patients who are housebound. Thisincludes palliative care, wound care, catheter care and phlebotomy. If you feelyou have the right skills to care for patients in their home environment, thenwe would like to hear from you. You must be willing to work within anintegrated team and develop your skills and knowledge. Community nursing isevolving at a fast pace, and you must be able to embrace change.
Main duties of the job
The Nursing Associate will assist in theidentification of health needs and participate in the delivery and evaluationof care interventions developed to meet those needs under the supervision of aregistered Practitioner. Supervision may be direct/indirect depending uponcompetency/complexity of work. The postholder will continue to develop and undertake a range of delegated tasks andwill report to a Registered Practitioner.

Job responsibilities
Function as an effective member of the DistrictNursing and wider multi-disciplinary team
Maintain clear concise and legible documentationadhering to prescribed standards and protocols
Use IT systems for relevant tasks, accessingpatient records & results, requesting investigations, data collections
Obtain consent in accordance with Trust ConsentPolicy
Use safe handling techniques and promote patientmobility, utilising mobility aids and equipment where appropriate
To maintain a patient orientated approach towardscare delivery and rehabilitation
Undertake a designated range of patient assessments(according to protocol)
Plan and deliver high standards of care (inconjunction with relevant professionals where appropriate)
Implement and evaluate individual care plans,utilising relevant outcome measures
Undertake a designated range of interventions(according to protocol) including wound care, application of plaster of paris,health screening etc.
Recognise the need for referral and undertakeappropriate referrals to specialist nurses/ practitioners/services within theNHS, independent & voluntary sectors
Promote and provide health education specific tointermediate care and in line with national and local policies
Undertake, monitor & record the following basicobservations: -
Temperature, Blood pressure, Pulse, Respiration
Peak flow
with readings outside of normal parametersreferred/discussed appropriately within the clinical team
Monitor and record blood glucose levels utilisingblood glucose monitoring equipment, with readings outside of normal parametersreferred/discussed appropriately within the clinical team
Undertake and record urine testing as per Trustpolicy, with readings outside of normal parameters referred/discussedappropriately within the clinical team
Utilise clinical skills appropriately to enhancethe delivery of patient care, Venepuncture, removal clips/sutures
Implement and evaluate a plan of care for simplewound management, as directed by a registered practitioner
Monitor and care for patients with respiratoryconditions, to include:
Use of oxygen therapy
Positioning to aidexpectoration/breathing
