Social Prescribing Link Worker
We have an exciting opportunity for someone who would like to work in the community connecting people with local activities and services to help them improve their health and wellbeing.
The role holder will provide personalised support to individuals, their families, and carers to take control of their well-being, live independently and improve their health outcomes. Develop trusting relationships by giving people time and focus on what matters to them. Produce a personalised support plan to improve health and wellbeing. The role will require managing and prioritising your caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies.
If you have excellent listening and communication skills, are empathetic and non-judgmental, are willing to work independently and as part of a team, have good IT and record-keeping skills and are emotionally resilient this could be the perfect role for you. A full clean driving licence and use of a car are essential. Full training will be given.
The position will work with GP practices in Tendring and/or Colchester and at Colchester Hospital, initially fixed term until 31st May 2025, with flexible hours available. The starting salary per annum will be pro rata to £22,880.00 rising to £23,880 after 12 months of satisfactory service. 5.6 weeks holiday per annum in the first year.
