An exciting opportunity to join a new Drug and Alcohol Care Team (DACT) providing in-reach support to adult mental health inpatients in County Durham / Darlington. Aim of the team is to develop a holistic and proactive partnership approach to support those experiencing mental ill health and drug and/or alcohol issues.
Through reinvestment moneys from OHID able to engage a bank band 6 substance misuse nurse to work with the acute service in Durham, established Alcohol Care Team within the local hospital and community substance misuse services. Support ward staff to enable continuity of care for both opiate replacement therapies and alcohol dependent users. Advise on induction, continuation regimes and facilitating continuation of community detoxes once discharged. Promote engagement with substance misuse services within the SU local area.
Cover three hospitals in Durham, in charge of a small team of clinical staff and service user experience workers. The role will form liaison between acute and community services promoting recovery and engagement.
Provide professional advice, promote community provision to the service users to promote engagement. Deliver after care interventions and supporting service users on the ward and once back in the community.
The role will perform a level of education and training to share best practice and increase acute services of substance misuse needs and treatment options.
Provide professional advice to colleagues within the acute services.
Promote engagement with OST or alcohol treatments
Enhance delivery of community detox options
Promote after care medications and engagement with aftercare medications
Liaise between acute and community service to enhance service delivery
Promote alcohol knowledge and harm reduction with service users and professionals
Community detox preparation and engagement, with support from acute and community services.
