You will be responsible for assessing, planning, implementing, and monitoring physiotherapy treatment for Reablement adults in Norfolk, ensuring interventions are effective and progress is regularly evaluated. You will be accountable for maintaining high clinical care standards and will refer or escalate cases to appropriate staff, agencies, or stakeholders when necessary. Your role includes supporting health promotion, recommending aids to help service users achieve their reablement goals, and maintaining effective communication with service users, support workers, and team members. You will manage your time, workload, and equipment efficiently, meet company targets, and adhere to Ceras rights-based approach, prioritizing service user choice. Additionally, you must ensure the health and safety of yourself, service users, and colleagues.
