This post has been funded by Macmillan Cancer Support for a 24 month fixed term contract or a secondment opportunity. This post is an exciting opportunity to implement the Macmillan Information and Support service project across Northumbria Healthcare Trust.
The post holder will manage, evaluate and continue to develop the service for the new service, supporting clinical teams and linking with established support sectors, primary care, public health, palliative care, acute teams and community services on the personalised care agenda in cancer and oncology care. Fundamental will be patient engagement at all aspects of this development.
Underpinning this is the living with and beyond cancer agenda, reducing health inequalities and ensuring patients/families can receive signposting to local support services. The service will utilise digital options, and the necessity for an agile service to meet the users' needs.
Main duties of the job
* Be able to work independently - manage the service.

* Knowledge of public health, cancer pathways and living with beyond/personalised care agenda, third sector, community services and reducing health inequalities.
Job responsibilities
* To manage and lead the Information and support team.
* Lead on the local implementation of the Macmillan Information and Support Service to improve quality of life for people living with and beyond cancer across primary and secondary care settings.
* Increase informational support to patients to live well after treatment (secondary prevention) utilising existing support services, local and third sector agencies, through the development of this Service.
* Increase patient experience of their cancer care in primary and secondary.
* Ensure consistency of care across Northumberland and North Tyneside, working with primary care, secondary care (cancer teams) in delivering Personalised care agenda (health needs assessment, treatment summaries, GP Cancer reviews, well-being events and information support).
* Support and bring about system change, implementing the agenda, as dictated by national and local drivers.
* Support on the development, scoping, mapping, co-ordination, implementation of the Macmillan Information Services for cancer patients across the trusts footprint.
Person Specification
Qualifications
* Degree or equivalent professional qualification of working in health & social care environment.
* Personalised care and/or public health skills and knowledge acquired through specialist training or demonstrable experience and knowledge of the personalised care agendas.
* Public Health or related Post Graduate Masters Qualification.
Experience & Knowledge
* Expert specialist knowledge, and clinical skills and experience in cancer pathways, personalised care or demonstrable experience in a cancer related post.
* Experience in quality improvement projects.
* Counselling skills.
Skills and Abilities
* Audit and research skills.
Other Requirements
* It is an essential requirement of the role that the post holder has a valid driving license and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £46,148 to £52,809 a year Pro Rata, Per Annum.
