Job Title: Team Manager- County and Younger Adults teams Contract Length - 3 months Location: Plough Lane, Hereford Hours: 37 Start: 18 Feb 2025 Target rate: £29.29/h The role Herefordshire Adults & Communities are developing adult social care services in keeping with the responsibilities under the Care Act 2014. We are looking for an experienced Manager to lead the County and Younger Adults Team Team to ensure continual improvement in hospital discharge practice, process and improved outcomes for individuals. The County Team work with individuals with complex needs such as Learning Disability, Mental Health, Forensic and/or a combination of these needs. Our Younger Adults Team works with Adults under 25 years of age who may be Care Leavers and are preparing for adulthood, the team work with those in receipt of complex needs funding and Education, Health and Care Plans. Both of the teams work with individuals in receipt of S117 aftercare, Joint CHC funding and work towards facilitating safe hospital discharges from both Community and Mental Health Hospitals. The successful applicant will have demonstrable management experience working in Social Care Teams, particularly with complex needs. The role involves close partnership working with various partners including health, adult social care, commissioning and Hoople to ensure a robust, timely and effective discharge for individuals coming out of hospital.
