An opportunity has arisen for a Social Worker to join Peterborough City Council’s 18-25 Transition Team.
We are seeking a Social Worker, who is eager to progress and be part of a service, that provides the best outcomes for young people when they are transitioning from children to adults.
This is an opportunity for you to be part of a dynamic team who support young people and their families from the first point of contact, all the way through to adulthood. This role is based within the Peterborough area, and our team is working within a hybrid model that enables working from home as well as being based at Sand Martin House, Peterborough.
We believe in giving young people and their families choice, control and flexibility in how they manage their support needs, and the right to access inclusive services and be a part of their own community. Social workers will be expected to use a Person-Centred Planning approach in seeking the views, wishes and feelings of the young people we work with.
The role involves completing Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) identifying individuals needs and supporting the Multidisciplinary teams’ recommendation on eligibility for full NHS Continuing Healthcare.
The successful candidate will have experience and/or knowledge of supporting people who are undergoing Transitions from children to adults, Experience and/or knowledge of The Care Act 2014, 0 -25 Special Education Needs and Disability (SEND Agenda) Community Support, the National Framework for Children and Young People’s Continuing Care (2016), and Children’s Act 1989 where young people are transitioning into adulthood. The successful candidate will assist families to access activities and services, Advice, advocating and will need to be able to travel independently between locations including out of county areas on occasions.
The 18 – 25 Transitions Team sits within the Adult Social Care workforce, with some joint working with CWD Team with young people from the age of 16 years old in Preparation for Adulthood.
