Integrated Community Health Worker – Brighton and Waltham

The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them. The Integrated Community Health Worker will serve as a frontline public health worker and as a liaison between health and social services and the community. The CHW will facilitate access to services and improve the quality and cultural competence of service delivery. If you have a calming presence, excel at understanding patient needs, and are willing to work on educating patients about the importance of health screenings, we would like to have you as part of our team. The CHW will work as a team member at our health center and will interact with patients and staff who are racially, ethnically, and culturally diverse, so being culturally competent is required.
YOUR RESPONSIBILITIES
* Establishes trusting relationships with patients and their families while providing general support and encouragement.
* Provides ongoing follow up, basic motivational interviewing, and goal setting with patients/families.
* Serves as a liaison between clinical staff and patient by following up with patients regarding their appointments, referrals, and needed resources.
* Helps patients set personal goals and attend appointments.
* Provides referral for services to community agencies as appropriate.
* Helps patients connect with transportation resources and needed resources to carry out the plan of care (Meals on Wheels, home health, Prescription assistance, etc.).
* Works closely with the integrated medical team to ensure that patients have comprehensive and coordinated care.
* Remains knowledgeable about community resources appropriate to the needs of patient/family.
* Continuously validates and evaluates the effectiveness of community resources.
* Provides consistent communication to Medical Provider/Nurse to evaluate patient/family status and reports progress and compliance with the identified plan of care.
* Enables and improves culturally and linguistically competent services and care by making efforts to understand the community culture and resources.
* Conducts outreach and implements programs in the community that promote, maintain, and improve community health needs and provide resources.
* Collects data to help identify community health needs.
* Advises community groups on chronic disease and diabetes self-management, hypertension, and nutrition.
* Advises clients on general health and exercise and self-care activities.
* Identifies age-appropriate needs and resources.
* Follows up on hospitalizations and no shows.
* Provides situation-appropriate social support to the patient utilizing experience lived and learned through training provided at CRCH.
* Provides informal counseling as needed.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS
* Bachelor's Degree in a related field required.
* Bilingual speaking preferred in Portuguese, Spanish, or Haitian Creole, but not required.
* BLS OR CPR Certification required, must be completed prior to start date, and must be hybrid or practical certificate including adult, pediatric, and AED trainings.
* Community Health Worker Mass Board certification preferred, not required. Must obtain within 3-4 years of hire date.
* Preferred Medical Interpreter Certification.
* Must have knowledge of computer systems such as Word and Excel.
* Strong interpersonal, organizational, and time management skills required.
* Must have a willingness to work flexible hours to meet the organization's needs/demands.
* Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
* Must have experience in diverse cultures, with a strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
* Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
* Knowledge of both community and healthcare system resources.
* Experience serving the population(s) that will be referred for patient navigation services.
* Excellent written and oral communication skills.
