Community Healthcare Support Worker - CRT The Community Response Team supports discharges from the acute hospitals, providing short-term and interim care. The team will also provide re-enablement to suitable patients to improve independence. Previous caring experience is ESSENTIAL. You will be based at Babington Hospital in Belper to cover the localities of Amber Valley and Erewash. You will need to be able to travel between visits, therefore a full UK driving licence and access to a vehicle is essential. You will need to be flexible and committed to caring and supporting people at home with care needs. Shifts follow a 7 day rota, including weekends and bank holidays.
