Main duties of the job The post holder will be an experienced Healthcare Assistant who will support in managing specific cohorts of patients with a range of acute and chronic medical conditions. They will work as part of the team, delivering care within their scope of practice to patients within the surgery and conducting home visits. They will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. They will carry out baseline observations such as pulse oximetry, blood pressure, phlebotomy, BMI, temperature as directed and record findings accurately. They will support with management of chronic disease, delivering opportunistic health promotion where appropriate. The post holder may complete basis care plans. This role is initially offered on a 12 month contractual basis, further extensions to be discussed.A travel allowance will be paid for completing home visits About us The role will support five practices within the Glossop area, with approx. 33,350 patients.
