Barking and Dagenham ICT community service is a forward-thinking and innovative service that works in a fully integrated way, with physiotherapists, occupational therapists, district nurses and community matrons all working side by side to ensure an integrated and holistic approach to the people accessing the community services.
Our Community Rehab Team assesses housebound adults with complex needs within their home environment. By assessing people within their home environment, we are in a good position to significantly improve people's quality of life and promote independence at home by providing therapy, therefore making this a very rewarding job.
Applicants should have post-graduate experience within the NHS encompassing a wide range of clinical experience. Previous experience in a community care setting is an asset.

Main duties of the job
Demonstrate flexibility, good communication skills and ability to work as a team member.
The ability to travel efficiently across the geographical area covered by NELFT is essential.
To prioritise workload, delegate and to supervise junior staff, support staff and students, to aid provision of high standards of patient care.
To work as an autonomous practitioner in the community, liaising closely with other members of the team, and other services as appropriate.
To carry out specialised Physiotherapy assessments and interventions for patients with complex needs.
To establish, evaluate and modify appropriate treatment plans, recognising own limitations and seeking advice when necessary.
Managing a caseload of patients using evidence based practice, the principles of Physiotherapy and patient centred practice.
To monitor, evaluate and modify treatment for patients with multiple conditions in order to measure progress and ensure effectiveness of treatment.
Working for our organisation
Starting with NELFT: NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period: This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement - London: This post attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata).

Experience
* 24 months post registration experience as a Physiotherapist in a relevant clinical setting in the NHS
* Experience in community adult community and rehab

* Car driver and has access to a car
