The post is a permanent position within a Multi-Disciplinary Mental Health Community Team which provides high quality Mental Health services across Portsmouth to people 65 years and over. The post holder will be required to carry a complex caseload and deliver a range of social work interventions, including high quality Care Act assessments and provision of services, Section 42 safeguarding enquiries, Mental Capacity Assessments/Best Interest, proactive case management and signposting to appropriate partner agencies. Section 117 reviews. As Band 6 social worker the candidate will need to be registered with Social Work England. Main duties of the job To provide a high standard of care for people in the care group, in keeping with the philosophy, operational policies for OPMH and in addition to the Trust's policies, practice and procedures. To be responsible for the gate keeping of all in-patient admissions into the Acute Pathway. To facilitate the early discharge from in-patient wards supporting the Hospital at Home team. To link with other services who refer into OPMH. To conduct accurate risk assessments and management plans, involving the service user, carer, and any other individuals relevant to the care of the service user, and to ensure that this is well documented. To carry out Assessments of clients meeting defined referral criteria as per local OPMH Service policies. To plan, implement and evaluate programmes of care for clients with acute mental health problems in the community. To develop strong links and good working relationships with REMIND, Adult Social Care and other mental health teams. To arrange Mental Health Act assessments.
