We are currently recruiting for a further Lead Reablement workerbased in Signet Court, Cambridge. The Reablement Service provides care at home to people aged 18 upwards. Support is delivered in an enabling way so that people can maximise their independence. Reablement support is a short term service. The Lead Reablement workers play a crucial role in supporting individuals transitioning towards independent living. They are the first point of contact for service users, working closely with Occupational Therapy teams and other health professionals to set personalized goals. These goals guide the service user through their journey until they achieve independence. Additionally, Lead Reablement workers act as key contacts, referring users to community support services and continuously monitoring their care throughout the reablement process. Our Lead Reablement workers support adults to maintain and regain independence while living at home. You will be responsible for agreeing goals with our service users and reviewing their progress, in their own homes. Most, but not all, of the people we provide service to are older adults who may have been unwell or are struggling to maintain their independence at home. You will work alongside Support Workers, with Social Workers and receive support from Occupational Therapists. About you We treat all of our service users as individuals and put their needs at the heart of what we do. You will have some experience in this kind of work. You will need to have a proficient level of written and spoken English for the purposes of case recording as well as an NVQ Level 3 or equivalent apprenticeship.Strong communication skills are essential as you will be overseeing Support workers in the community setting and liaising with many health professionals as well as our service users and their families. As a Lead Reablement worker you will need to have experience of supporting aperson with care and support needs as well as knowledge around The Care Act and CQC requirements. You will need excellent verbal and written communication skills as well asIT skills. It is expected that you will also have the ability to demonstrate awareness and understanding of equality, diversity and inclusion as well asthe need for safety and confidentiality in the role. The role involves travel across Cambridgeshire, and so you will need access to a car, and have a full driving licence with business insurance. You will receiveexcellent training and supervision as part of an established and supportive team. There is a car lease scheme and pay enhancements for weekend working. The service covers seven days and so there will be some weekend working.
