Working within Eden's GP practices and closely with the wider ICC team, the Living Well Coach (LWC) will work within the Wellbeing Service to assess individuals with LTC, Frailty, and psycho-social needs, engaging them to set personalized goals and access services best suited to their circumstances.
The assessment may include basic health assessment, low-level clinical screening, wellbeing assessment, review of functional abilities, and understanding social and support networks.
LWCs will develop skills in active listening, motivational interviewing, and psychological care skills, including CBT, acceptance & commitment therapy, and mindfulness. Training and ongoing clinical supervision will be provided by the Physical Health Psychology Department within NCIC.
Main duties of the job
Using their coaching skills, LWCs support individuals identified by GP practices and the wider Integrated Care Community (ICC) to improve self-management of their health and wellbeing. The coach will listen to what is important to the person and help them commit to step-by-step activities towards their improvement goal.
Although based within one or two GP practices in the Eden ICC, the post-holder will work in various environments, including visiting people in their homes or other community health/care settings. LWCs will be allocated to one or two practices to ensure continuity for patients and direct engagement with the GP practice team.

Experience
* Experience of working within a team
* Experience of organizing own day-to-day work and adjusting priorities

Qualifications
* A qualification in Health/Social Care or an accredited award such as NVQ Level 3 or equivalent experience
* Care Certificate or willingness and ability to undertake it
* ECDL or equivalent
