Job summary We are recruiting to our homeless and complex needs team to provide outreach services to patients in hostels and in practice who require support with their health needs, we are an award winning team that comprises of GPs and Nurses who provide holistic care to this complex group. Main duties of the job To provide long term condition management Ensure all vaccinations and immunisations are up to date and assess the patients risks and vaccinate appropriately Assessments in hostels and in Practice at HAC (homeless access clinic) Liaise with other agencies to ensure we are providing the best outcomes for this client group About us Brownlow prides itself in being an exceptional employer.
