Job overview
The post holder will act as the point of contact for the Stoma CNS nursing service across site and would be responsible for triaging calls and giving basic advice. They must have good communication skills which are essential, and they will proactively identify and take action to ensure that patients experience a seamless pathway.
The role involves being responsible for maintaining accurate patient records and liaising across organisational boundaries to assist the Clinical Nurse Specialist to develop the nursing service. They will also provide the highest quality patient care.
Main duties of the job
* Act as a main point of contact for patients, carers and wider health care teams. Triaging incoming calls and initiate appropriate responses, using a level of clinical knowledge according to protocols and individual pathways.
* Receive highly complex and demanding information, deal with distressing situations in a sympathetic, calm, and supportive manner.
* Proactively identify situations where dates of diagnostic tests, treatment and clinic review may not be scheduled appropriately.
* Manage own caseload making pre-planned outbound telephone calls to patients to assess needs, plan care and monitor effectiveness of services and patients’ self-management, triaging as appropriate.
* Maintain accurate and concise records of patient contacts (paper/electronic) to ensure seamless communication pathways for the wider healthcare team.
* Compile and collate Stoma service and patient information for databases and audit purpose.
* Liaise across organisational boundaries and work with ward staff and teams regarding issues involved in patients’ care.
* Undertake unexpected and demanding duties in an efficient, timely and cooperative manner.
* Act as patient advocate and facilitator to resolve issues that may be perceived as barriers to care.
* Assist the clinical nurse specialist to develop education and information resources for the public and other professionals regarding all aspects of cancer.
* Refer patients to other care providers.
* Liaise with various companies and services in ordering supplies.

Person specification
Qualifications
* Educated to GCSE/O’ level standard in core subjects or evidence of working at that level.
* Computer literate with knowledge of IT systems such as Word and Excel.
* HNC or NVQ Level 3 or equivalent evidence of working at that level.
* Knowledge of databases and spreadsheets.
Experience
* Minimum 2 years experience within the NHS or similar environment (e.g. Dept of Health or other public service).
* Experience of working with Stoma patients or within a relevant clinical setting.
* Experience of working with multi professional teams.
* Evidence of ability to develop effective working relationships with different levels and disciplines internal and external to the organisation.
* Experience of managing resources.
* Experience of audit and data collection.
* Administrative support for clinical team including use of Microsoft office and experience of patient information systems (CPD).
* Experience of telephone triage and patient contact.
