They will support general practice staff with prescription and medication queries, assist with repeat prescribing, handle acute prescription requests, and perform medicines reconciliation for patients post-hospital discharge. They will provide expert advice on medicines to patients and the multidisciplinary teams. Responsibilities include leading medicines optimisation, quality improvement, and overseeing aspects of the quality and outcomes framework and enhanced services across all sites. The post holder will also facilitate integration with community and hospital pharmacies to enhance patient care. This role is key in improving care quality and operational efficiency, requiring motivation and passion to deliver excellent services. The post holder will be supported to develop into a non-medical prescriber as needed. They will work independently and as part of the Pharmacy Team, recognising the roles of colleagues and other stakeholders, including patients, senior and peer colleagues, and professionals from other organisations (e.g., CCGs). The post holder must acknowledge their limitations and refer to others when necessary.
