Our Reablement Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans. As a First Responder, you will be to support with Hospital discharges, and be first point of contact for new customers, assessing their needs, compiling a support plan, and attending to needs such as personal care, food preparation and daily living activities as required. To meet the needs of the service, shifts include early morning, evenings, weekends, and bank holidays, traveling across the Wiltshire area to carry out customer visits. You will work alongside Occupational Therapists and carry out trusted assessments.
