Job summary
An opportunity has arisen for a new Practice Nurse to join our friendly team.
We are looking for a practice nurse to work 2 full days a week plus one Saturday per month. Working days available within our practice are Tuesday and Friday.
Main duties of the job
The successful applicant would be expected to provide nursing services to the practice population with evidence based, quality focused care. To assess, plan, implement and evaluate care. To ensure clear channels of communication exist between clinical, administrative and community teams. To project a positive and friendly image to our patients and other professionals. To lead infection control policy & process within our practice and ensure the practice is compliant with CQC IPC requirements.
About us
Pentelow Practice is located in Feltham, TW14 9EF and is a member of the Feltham and Bedfont Primary Care Network (FAB PCN). The practice has a list size of 4000 patients. Staff comprise of 4 GPs, 1 Practice nurse (current vacancy), 1 Health Care Assistant and 4 Receptionists. The practice uses SystmOne.
FAB PCN provides additional clinical and administrative staff to support the practice (Clinical Pharmacist, Pharmacy Technician, First Contact Physiotherapist, Mental Health Nurse, Paramedic, Social Prescribing Link Worker and Care Coordinator)
Job description
Job responsibilities
1. Baby Immunisation
2. Wound Check
3. Smear Test
4. ECGs
5. COPD reviews
6. To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems
7. Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
8. Provide travel health advice and administer appropriate vaccinations
9. Treat patients with minor injuries and provide wound care.
10. Provide directly and assist GPs with contraceptive services
11. Communicate with Community to achieve best patient outcome.
12. Act as chaperone when required
13. Undertake health promotion to help educate patients
14. Undertake regular infection control audits and act as an infection control lead.
15. Undertake anticoagulation clinics to include interpretation of blood tests and dosing (in the event of absence of other member of staff)
16. Act as IPC lead for the practice
17. Responsible for developing & implementing all policies & procedures relating to the nursing team & IPC.
18. Provide oversight for the HCAs
