One You Lifestyle Adviser (Primary Care)

Have you got what it takes to be One You Lifestyle Advisor within Primary Care? Are you passionate about health and willing to help other people to make the change to a healthier lifestyle? One You Lifestyle Advisors work in their local communities, via GP surgeries, to help people who need a bit of extra support to make the change to a healthier lifestyle. We provide personal help and support to people on a range of issues including: Healthy eating, Exercise, Smoking, Emotional wellbeing, and Advice on accessing local services.
We need enthusiastic people to join our team! You don't need any formal qualifications, but you must be passionate about health and able to use your local knowledge and great communication skills to help people in your local community. We have 1 part-time (22.5 hours per week) position available in the Canterbury and Coastal locality, with the scope to cover Canterbury, Faversham, Whitstable, Herne Bay, Ash, and Sandwich.

Main duties of the job
One YOU Lifestyle Advisers (Primary Care) are expected to be kind and responsive but professional and informative and contribute to the quality of the services provided within the community team across the health economy by:
1. Contributing to the ongoing assessment, planning, implementation, and evaluation of delegated caseload and recording all outcomes on the appropriate systems seeking the support of senior/specialist staff as required or to support re-assessment.
2. To undertake, and report back on delegated activities that have previously been signed competent, e.g., height, weight, and blood pressure.
3. To undertake a variety of clinical intervention tasks after achieving appropriate competencies delegated by a senior staff, e.g., Health MOTs.
4. Providing quality advice and guidance in relation to lifestyles/health improvement to support goal setting and self-management within the scope of expertise.

Person Specification
Qualifications
* Level 3 qualification (A-Level, City & Guilds Level 3/ NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) and/or equivalent experience.
* Functional Skills at Level 2 (Literacy and numeracy/GCSE grade A-C or equivalent in English and Maths).
* Further related CPD; i.e., nutrition, physical activity.
* RSPH level 2 in Understanding Behaviour Change and/or Understanding Health Improvement.
Experience
* Experience of working on own initiative.
* Work (paid or voluntary) in a community setting.
* 1 years' experience in a health or social care setting.
* At least two years' experience working in a health improvement setting, including experience within community settings.
* Handling a range of public, client, professional enquiries.
* Working in a busy environment.
Knowledge
* Knowledge and experience of caring for, and empowering others respectfully.
* Basic knowledge of the principles of health promotion/improvement/inequalities/risk and the wider determinants of health.
* Know how to find out about local services and how to support individuals to use them.
Skills and abilities
* Good communication and interpersonal skills (verbal and written).
* Excellent time management skills and the ability to organise own work and use own initiative.
* Be able to travel to locations across Kent in a timely and efficient manner.
* Good IT skills.
* Be able to account for and manage own resources and those of others associated to the service and have an awareness of own limitations.
