CLINICAL 1 To be professionally accountable and responsible for all aspects of own work. 2 To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values. 3 To triage new referrals identifying clinical risk, making an assessment of CYP and formulation of needs, with ingoing assessment, planning implementation an evaluation of programmes of treatment/support for children, young people, families and carers. Takes a lead in specialist area and provides advice and support to others. The specialist care/support plans may include the following: Promoting independence/self-management and maintaining safety Increased functional ability Improvement in health and wellbeing including mental health Interventions in condition management Prevention and reducing of health inequalities Case management/ treatment /care plans 4 To use specialist knowledge and judgement to take into account the range of support packages and treatment options available. To consider the personal circumstances and preferences of children, young people and families in order to implement and adjust programmes of care and intervention. 5 To maintain and develop skills and knowledge to deliver a range of interventions and set an example of good practice to other team members including skill sharing. Provides teaching/training in specialist area to the wider workforce as required. 6 To maintain sensitivity at all times to the emotional needs of the children, young people, families and their carers offering reassurance and empathy, in particular when imparting potentially distressing information. This may be in regard to the nature/prognosis of their condition/situation and implications of the same including managing any conflict that may arise. 7 To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed. 8 To use specialist professional knowledge, skills and expertise when working in changing environments and across professional boundaries. 9 To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with professional standards (e.g. NMC). Support identified team members, giving guidance and direction where appropriate. This may include a variety of MDT members and student nurses. 10 To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques, and a flexible approach to responding to the needs if the CYP and families. 11 To provide advice, education and training to service users, carers and professionals regarding the management and care/support of service users. 12 To contribute to professional, clinical support and leadership. Offer support and guidance in identifying training needs of staff. 13 To be conduct and be accountable for assessments, planning and implementation of care/treatment packages/ programmes within their caseload as part of a MDT. 14 To demonstrate a high level of clinical judgment, acting autonomously in a variety of contexts. 15 To contribute to risk assessment and management in all areas of clinical practice including any safeguarding issues following relevant Trust policies and procedures Supervision 1 To provide, receive and encourage clinical supervision in order to support the continuous professional development of self and others (and any other managerial and professional supervision where required). To Maintain and assure a high quality standard of evidence based practice. 2 To contribute to the process of induction for new team members and other disciplines, as appropriate. 3 To identify own training/educational needs as part of the supervision and appraisal process. Attains and maintains an agreed level of expertise through ongoing training and development. 4 To have people management skills which incorporate compassionate and appreciative leadership. 5 To set workload priorities for self, and oversee the workload of more junior team members and students as required. 6 To provide training to students as required and to be up to date and current with all support of student nurse process, such as advisor and assessor role. Acts as preceptor to newly appointed staff, and staff undergoing a supervised return to practice. 7 To undertake tasks concerning the day to day organisation and operation of the team working with the team leader as required. 8 To set priorities to achieve a balance between multi-disciplinary assessment and profession specific caseload work. 9 In collaboration with the team leader, establishes clear goals and standards for the team/service in order to maintain and improve the quality of care/support. 10 To work with the Team Leader/ Nurse Professional Lead to identify clinical levels of expertise, knowledge, skills and competence within the team, in order that talents, training and competences are identified and are fully utilised. Performance & Quality 1 To collect, correlate and input data as required within agreed timescales. 2 To take the lead in clinical audit/research for specialist area and participate in and support the team in undertaking clinical audit. To contribute to trust wide, local and national research and audit as deemed appropriate. 3 To contribute to, and actively promote the implementation, development and upholding of policies, procedures, guidelines and directives, along with relevant statutory measures and governmental directives. 4 The completion, collection, interpretation of recognised outcome measures to ensure effective care, treatment and service evaluation.
