Purpose of Role
To work as an integral part of the Care Home team, positively promoting recovery and social inclusion by focusing on the needs of Service Users on either a 1:1 or 2:1 basis. Contribute to the ongoing support of Service Users, promoting their rights, independence, individuality, and physical and mental wellbeing. Assist with the rehabilitation of Service Users, including activities of daily living, household tasks, sport, social, and leisure activities.
Key Responsibilities
1. Assist in all areas of care in accordance with agreed care plans and procedures, enabling Service Users to meet their individual needs.
2. Administer prescribed medication as needed.
3. Contribute to the development and evaluation of care plans under the supervision of a Registered Nurse.
4. Contribute to the prevention and management of behavior that challenges by employing positive behavioral support approaches.
5. Form positive therapeutic relationships that support Service User’s decision-making and build on their strengths, encouraging community activities.
6. Conduct, record, and report observations to ensure care plans are implemented and quality standards are achieved.
7. Monitor the wellbeing of Service Users, including physical health checks.
8. Encourage and support individuals to participate in meaningful community activities.
9. Facilitate/co-facilitate groups and activities.
Duties and Specific Responsibilities
Manage the Safety and Quality of the Service
1. Promote the delivery of a safe service in accordance with legislative requirements and Company Policies.
2. Report any illness of an infectious nature or accidents incurred by Service Users, colleagues, or self.
3. Understand and adhere to the implementation of the Company’s Health & Safety Policy.
4. Report any faulty appliances or potential hazards.
5. Implement effective practices that support compliance with health & safety at work.
6. Maintain accurate records of Service User support under supervision.
7. Adhere to the Policies and Procedures of the Company.
8. Report all complaints and incidents to the Nurse in Charge.
9. Be prepared to work flexibly to ensure the safety of service delivery.
Provide High Quality Service Provision
1. Promote the rights of each Service User and keep their wishes at the center of their care.
2. Participate in pre-admission transition plans to support effective admissions.
3. Support Service Users to participate in activities that meet their needs.
4. Support Service Users and their representatives to understand service provision.
5. Apply effective communication skills with Service Users, their families, and other professionals.
6. Keep all information about Service Users secure and confidential.
7. Support the delivery of service actions aimed at promoting quality service provision.
Leadership and Management
1. Complete required social care training to fulfill the role effectively.
2. Support the identification of ongoing staff training needs.
3. Participate in planned supervision and appraisals.
4. Comply with Safeguarding Adults legislation and report concerns.
Promote the Business
1. Handle inquiries regarding potential placements professionally.
General
1. Contribute to the development of an effective working team.
2. Develop and maintain a network of professional contacts.
3. Report issues that could bring the Company into disrepute.
4. Maintain confidentiality of information.
5. Notify Line Manager of absence in a timely manner.
6. Ensure security of the work location is maintained.
7. Ensure equipment is clean and well maintained.
8. Carry out any other reasonable tasks assigned.
9. Ensure resources are conserved sensibly.
10. Ensure compliance with contracts and quality-related payments.
Dimensions
The post-holder will support Service Users to live fulfilling, safe, and meaningful lives, contributing to the reviews of care plans and documentation.
Working Relationships
The post-holder will develop good working relationships with staff, management, visitors, and other professionals.
Experience and Qualifications
* NVQ Level 2 or 3 in Health and Social Care essential, or committed to undertake NVQ/Certificate in Mental Health.
* Minimum of 1 year’s experience in a Care Home setting with complex mental health and social care needs.
* Experience promoting social inclusion and recovery.
* Ability to form and maintain positive therapeutic relationships.
* Ability to drive a work vehicle or use own vehicle.
* Experience supporting vulnerable adults in health and social care services.
Working Hours
5 days over 7, Monday-Sunday, 10.00-18.00, with occasional 9.00-17.00 required.
