An exciting opportunity has arisen for an enthusiastic, experienced orthopaedic nurse to join our specialist multidisciplinary COPE Team. As part of the Trauma and Orthopaedic Unit, this hospital-based community team supports early and same day discharge of patients from hospital, providing a continuation of hospital-based care and rehabilitation in their home environment.
The post involves post-operative 6-week assessments of primary hip and knee replacement patients in their homes, reducing the need for hospital visits. The team is both interdisciplinary and multidisciplinary, allowing you to extend your scope of practice following training and competencies.

We are looking for an innovative individual who has excellent communication, team working, and clinical skills. Successful candidates must have a relevant clinical background and the ability to integrate well into a multidisciplinary team environment.
Key Responsibilities:
1. Provide specialist assessment, treatment, and advice to patients within the community setting, facilitating and maximizing patient potential following orthopaedic surgery.
2. Ensure all care is provided in accordance with BHR trust policies and procedures while aiming to achieve high level care set by national and departmental guidelines.
3. Give specialized guidance to other members of the MDT and GPs to provide a seamless approach to patient care, aiming to support discharge from hospital and prevent re-admission.
4. Provide support for the rehabilitation team, in conjunction with the COPE Team Lead, in planning, coordinating, delivering, and evaluating the care provided to patients within your specialist area on a day-to-day basis.
