Job summary We are looking for an enthusiastic and driven individual to become a member of the Site Management Team as Patient Flow Assistant on a permanent basis. You will assist and provide requested support within all areas of patient flow enabling the hospital sites to provide efficient and effective patient flow helping to reduce barriers to patient flow and supporting the Trust in providing a gold standard service enhancing our patients experience. The Patient Flow Assistant will be expected to work as part of the Site Management Team under the direction of the Patient Flow Manager, to ensure the smooth flow of patients through the wards and departments across the hospital. It is expected that by co-ordinating administrative tasks, and liaising with other departments and the site team, the post holder will contribute to efficient and effective patient flow, resulting in a significant reduction in patient journey times. The post holder will be required to assist the Patient Flow Managers within the Site Management Team to plan and facilitate a timely discharge process for patients from point of admission to the ward environment to the discharge of patients to their own home or alternative care settings. You will have excellent communication and interpersonal skills to ensure close working relationships exist within the acute setting at various management and clinical levels and with key partners across the Health and Social Care networks.
