An exciting opportunity has arisen for an experienced, motivated and enthusiastic nurse to join the cardiac adolescent and transition service. We are looking for an experienced cardiac nurse with vision and enthusiasm for delivering holistic young person centred care. The role includes service development of transitional care for young people with congenital and acquired heart disease, leading a small, motivated team, whilst managing a caseload of young people with congenital heart disease (CHD) and acquired heart disease. You must have excellent leadership and management skills, experience in service development and improvement and aspire to develop innovative teaching strategies, whilst also being able to quickly develop a positive rapport with young people. The role will involve developing strategies to empower young people to manage their on-going health needs as they prepare for adulthood, whilst implementing pathways to reduce patients becoming lost-to-follow-up. We will support your on-going development and educational pathway to achieve these goals. This is a full-time position. Main duties of the job Work with cardiac services to develop a network of care including inpatient, outpatient and community support whilst demonstrating a nurse lead in this area of practice Work with young people with congenital heart disease (CHD) and acquired heart disease aged 12 years upwards within the Trust, across the cardiac networks, and into the community Demonstrate appropriate expert clinical practice and knowledge of this patient group, leadership skills, education, and an ability to use research/audit to develop practice and improve current knowledge base Develop clinical judgment and decision making in relation to the management of patients.
