We are seeking an enthusiastic Nurse, Paramedic, OT orPhysiotherapist to expand our home visiting team. A clinician with a special interest in frailty and older people, supporting delivery of same day care in the community.

Main duties of the job
Main duties
a)Undertake same day urgent visits, prioritising in order ofclinical need, acknowledging this can change at short notice due to servicedemand.
b)Undertake comprehensive geriatric assessments includingmedication reviews and advanced care planning where appropriate.
Take an accurate history, undertake suitable systems examinationincluding observations and other point of care tests as appropriate.
Diagnose and manage acute and chronic conditions.
Utilise and refer to community services where appropriate, ensuringpatient safety throughout.
Communicate findings and management plan with patient/relatives andreferring clinician.
Provide safety netting/ worsening advice for patients and relatives.
Ensure comprehensive, timely and accurate documentation.

Job responsibilities
This role is for a clinician to support primary care as part ofour community care team working during the working day, Monday to Friday.
The post holder will provide holistic care for individuals intheir homes including assessment, diagnosis and management to provide the bestoutcomes for the patient.
The post holder will have the ability and confidence to safelyand comprehensively carry out a full systems assessment, autonomouslydischarging patients within your own scope of practice seeking senior colleagueadvice as required. It is important that you are comfortable working in thiscapacity though you will always have access to support from senior clinicalcolleagues at other sites.
The postholder must be able to refer and/or signpost patients intheir own home or care home to other allied health professionals for examplecommunity COPD nurses, Social prescribers etc.

Essential
1. Regulated Health Care professional with current registration: (NMC/HCPC Registered).
2. Minimum of three years post registration with demonstrable experience of using diagnostic skills
3. Postgraduate certificate or diploma (nurses only)

4. Recognised qualification in physical assessment and clinical reasoning or equivalent.

6. Access to own transport and ability to travel across the locality on a daily basis.
