Job summary
Building on Haxby Groups experience as a regional workforceand training hub alongside a vision to become an at-scale multiprofessionalprimary care training provider, this innovative role will allow qualifiedAdvanced Clinical Practitioners with an interest in education to maintain and develop existingclinical skills in a supportive practice environment alongside an expanding educationalportfolio. A proportion of the successfulcandidate's week will be spent working as part of the Haxby Group Trainingteam. This will involve the development and deliveryof teaching and leadership skills, helping to mentor and support both currentand aspiring practice nurse and other health professionals.
Main duties of the job
In the clinical part of the role you will be responsible for delivering quality holistic careand treatment to the Practice population. Whilst you are a dependent practitioner, youare able to work independently and make independent decisions enabled by acollaborative and supportive working relationship with your clinical supervisor. You willdemonstrate safe and effective clinical decision making and expert care, includingassessment, diagnostic and management skills. As well as dealing with acute illness,you may be involved in the management of long-term conditions.
You will be responsible for supporting, designing and delivering training programs thatwill meet the needs of our customers. You will be responsible for performing trainingneeds assessments, designing and delivering curricula and managing all phases oftraining interventions. You will support in the development of education and trainingopportunities, projects and schemes within the remit of Haxby Group Training and thePrimary Care Workforce and Training Hub (PCWTH), liaising with practices acrossHumber and North Yorkshire to provide support and guidance relating to students andtraining. You will be involved in directly supporting and developing other trainers withinthe wider training company to aid deliverance of multiple trainer programs accounting forour wider trainers offer. You will be involved in supervising other clinicians within ourlocal and regional training network.

Clinical Responsibilities:
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and carefor patients presenting with an undifferentiated diagnosis within your sphere ofcompetence
2. Clinically examine and assess patient needs from a physiological andpsychological perspective, and plan clinical care accordingly
3. Assess, diagnose, plan, implement and evaluate interventions/treatments forpatients with complex needs
4. Utilise your clinical supervisor or mentor and other clinical staff appropriately toensure safe and effective patient care, demonstrating awareness of your ownlimitations Primary Care
5. Provide safe, evidence-based, cost-effective, individualised patient care within thesurgery, patients own home or other environment where patient care is carriedout.
6. Refer patients directly to other services or agencies as appropriate, utilising Practiceand local guidelines and in a timely manner
7. Pro-actively identify, diagnose, monitor and manage treatment plans for patientsat risk of developing a long-term condition as appropriate
8. Diagnose and manage acute conditions, integrating both drug- and non-drugbased treatment methods into a management plan
9. Prescribe and review medication for therapeutic effectiveness, appropriate topatient needs and in accordance with evidence-based practice, and national andpractice protocols, and within own scope of practice. Utilise Patient GroupDirections for the supply of medication as appropriate.
10. Work with patients in order to support compliance with and adherence toprescribed treatments.

Teaching and Training Responsibilities:
16. Pro-actively working, leading when required, to support and develop the PrimaryCare Workforce and Training Hub (PCWTH) hosted by Haxby Group.
17. Support in the development of education and training opportunities, projects andschemes within the remit of Haxby Group Training and the PCWTH
18. Supporting with Practice Learning Facilitator work, liaising with practices andeducation providers across Humber and North Yorkshire to provide support andguidance relating to students and training.
19. Identifying and assessing future and current training opportunities and needs.
20. Translating requirements into training services and products that meetopportunities and customer needs.
21. Maintaining a keen understanding of training trends, developments and bestpractice.
22. Developing a training services plan and assist in building the annual trainingdelivery programme.
23. Train the trainers role: directly support and aid development of other trainers
24. Preparing or overseeing teaching plans.
25. Delivering training and courses, deploying a wide variety of training methodsdelivering content in keeping with relevant local and national evidence andguidance.
26. Monitoring and evaluating training programmes effectiveness, success and returnon investment and reporting on them.
27. Identifying and assessing future and current training needs through job analysis,career paths, annual performance appraisals and consultation with line managers.
