As a Community Support Worker, you will be a key member of our multi-disciplinary Home Treatment Team, focused on preventing hospital admissions and facilitating supported discharges from the hospital. You'll be working in a community mental health setting, evaluating and communicating clinical information with a range of teams and the Trust to ensure patient-centred care is provided at all times. The role will include direct and indirect care to service users, under minimal supervision and as directed by the Team Leader. Your role as a Community Support Worker will include assisting in reviews of service users in the community, contributing to the assessment of their needs, and ensuring care plans are fully implemented. You'll also provide comprehensive and efficient support to the clinicians and multi-disciplinary team, which will include updating records, discharge summaries, dealing with telephone enquiries, and initiating and sending out routine correspondence. You'll also be responsible for handling service user information in a confidential manner, in line with relevant policies. Additionally, you'll assist with the preparation and delivery of workshop materials, under the guidance of the clinical team.
The ability to travel is essential as the role is Sussex wide.,
* Have experience working with children and young people
* Be able to analyse and communicate complex information verbally and in writing
* Be able to prioritise workload according to young people's changing needs and the priorities of the team
* Be familiar with different IT system
It would be advantageous if you have:
* Emotional first aid or other emotional well-being basic training or understandin, A recognised qualification in an area related to the workplace or equivalent technical/life skills
* Evidence of a commitment to lifelong learning
* Health care or relevant NVQ level 3 or equivalent skills, Experience of working with people with mental health needs in a paid or voluntary capacity
* Experience of working with people with mental health difficulties
* Experience running groups/activities
* Experience working as part of a team
* Experience of being supervised
