Improve outcomes for people struggling to access wellbeing and community services by working with primary care clinical staff to provide short-term, targeted, and holistic support.
The Mental Health Support Coordinator (MHSC) Service works in close partnership with Primary and Secondary Clinical Services and other key providers in the area to ensure integrated pathways for clients and in particular clients who face more complex situations. The service will bring together the PCN, Southdown and Sussex Partnership NHS Foundation Trust, working in partnership through a multidisciplinary team approach.
You will be:
Providing short term support to a caseload of clients with complex needs utilising a strength based and person-centred approach to support them to engage and achieve their outcomes.
Improve patient wellbeing through increasing access to non-clinical services by using motivational & coaching techniques with patients.
Facilitating smooth pathways into services using advocacy, good communication, collaborative working with services.
Promote and support the implementation and development of the service by building and maintaining professional positive relationships in GP practices by attending GP, Multi-Disciplinary Team and SPFT clinical triage meetings where appropriate.
Using GP and partners IT systems to record and monitor client information and managing a caseload of patients.
To succeed in this role, you will need an excellent understanding of complex mental health needs and wellbeing with experience of supporting this client group orequivalent transferable skills and a genuine passion for supporting people to explore their needs. You will receive a comprehensive induction followed up with regular training we will invest in you to give you all the tools you need to achieve. The salary for this role will increase to £27,128 from 1st February 2025.
