We are excited to announce a new opportunity within our Hampshire and Isle of Wight All Age Continuing Care Mental Health and ABI Team. We are seeking self-motivated professionals who share our values of being person centred, honest, courageous, innovative, and collaborative and who are passionate about improving life opportunities for people who have existing or newly diagnosed mental health conditions and who require S117 aftercare to allow them to live full and meaningful lives in the community.
Main duties of the job
As a Senior Clinical Assessor/Practitioner in Mental Health and ABI Team, you will be responsible for routinely screening and approving funding applications for clients who require packages of care or placements that are eligible or have S117 aftercare entitlement under the Mental Health Act 1983. You will work closely with our Southern Health Care Coordinators and social services colleagues to ensure that commissioned care is safe, appropriate, and cost-effective through ongoing review and assessment.
You will enable timely provision of appropriate care and support ensuring that people's needs are effectively met whilst promoting Personal Health Budgets to encourage independence.
You will need to have excellent interpersonal, presentation and communication skills and an ability to make decisions in a challenging but supportive environment. As part of the role, you will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individuals' own homes and supported living environments.

Job description
Job responsibilities
1. Working alongside the clinical lead to screen, approve, review and case manage clients through to discharge - those requiring a dedicated period of specialist rehabilitation following an acquired brain injury.
2. Working alongside specialist providers, clients and families to ensure quality outcomes, managing expectations and preventing delays in transfer of care along the pathway.
3. Working knowledge of complex neurological conditions disease process and injury with respect to cognitive and physical impact required.
4. Experience of working with clients with complex brain injuries that may have new or pre-existing dual mental health conditions requiring care under the Mental Health Act.
5. Acquired Brain injury case management experience.
6. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/neurological dysfunction, e.g. NICE LTC.
7. Support to the mental health assessment pathway may be required in this post.
Person Specification
Knowledge, Training and Experience
Essential
* Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE, including AMHP's and mental health social workers or significant relevant experience (LD/MH/LDAP teams only).
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care.
* Experience of assessment of health and social care needs - care planning and coordination.
* Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles).
* Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/neurological dysfunction (Specialist Rehabilitation / ABI team).
* Ability to work effectively under stressful conditions in a rapidly changing environment.
* Demonstrable experience of care/case management.
* Knowledge of and experience of NHS Funded Continuing Health Care criteria - guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act).
* Good knowledge of Mental Capacity Act and relevant legislation.
* Experience in communications and stakeholder management.
* Effective working knowledge of Microsoft software.
* A good understanding of the health and social care environment and roles and responsibilities within it.
