Job responsibilities Responsible for providing and delivering primary health care interventions and care effectively and efficiently Working collaboratively with the multi-disciplinary general practice team and other healthcare professionals to promote integrated pathways of care to meet patient needs. Effective communication with patients, carers and all members of the MDT Effective prioritisation and completion of daily work in line with clinical / patient need and practice priorities Maintenance of accurate and legible patient records in line with professional and organisational requirements this includes contemporary paper records and electronic patient record system Actively participates and contributes to nurse team meetings Initiates health promotion activities with individuals, patients and carers Participates in clinical audit and the implementation of changes to ways of working and/or clinical practice as a result of findings, recommendations and action plans Proactive in improving and influencing the patient care pathway/ journey Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs. Such tasks may include: Basic wound care/ulcer care Removal of sutures NHS Health Checks Urinalysis ECGs Peak flow readings Venepuncture Blood pressure measurements ABPMs (organisation of fitting/downloading results) Ear syringing Routine immunisations (including childhood, travel) Venepuncture when required Chaperoning and assisting patients where appropriate who are being examined by another clinician Assisting GPs with minor surgery and coil/implant fittings or leading the coil/implant clinic independently Requesting basic pathology tests, for example urine culture, swabs Following agreed clinical protocols with referral to senior nurses or GPs as appropriate Reviews of Long term Conditions as per training ( e.g. Asthma, Diabetes ,COPD) Cervical Cytology Contraception and sexual health
