What You Will Do:
* Assessment and Care Planning: Carry out assessments for individuals with cognitive and physical disabilities, understand their unique needs, and develop person-centred care plans that help them achieve their personal goals.
* Service Coordination: Act as the key link, connecting individuals to vital services (healthcare, community resources), ensuring seamless support and improved quality of life.
* Support for Families and Carers: Offer guidance and support to families and carers, providing them with essential information and resources to assist in the care of their loved ones.
* Safeguarding and Risk Management: Take responsibility for safeguarding vulnerable individuals, identifying potential risks, and adhering to safeguarding protocols and legislation to ensure their safety and wellbeing.
* Collaboration with Multi-Disciplinary Teams: Work closely with health professionals, therapists, and other specialists to create a comprehensive care approach that addresses all aspects of cognitive and physical health.
