Devon Partnership NHS Trust form one of a number of keystakeholders in Torbay delivering innovative approaches to the support andmanagement of individuals with complex needs, including substance misuse andconcurrent mental health problems.

The role involves managing a definedcaseload of service users with a focus on those with a concurrent presentationof substance misuse and a severe and enduring mental health problem, ensuringeffective engagement and treatment delivery to address both presentations in acoordinated approach.
You will be required to coordinatetreatment packages that are personalised to the individuals treatment andrecovery needs. To do this effectivelyyou will be required to liaise with a wide range of statutory and non-statutoryagencies
You will be required to provide arange of evidence-based recovery focused interventions. You will be working with service users withcomplex issues related to their substance misuse including vulnerability,homelessness, criminal justice safeguarding issues and mental health issues.
Within the alliance the post holderwill be identified as the locality resource to develop links with mental healthservices and to develop resources for the wider team.
Main duties of the job
You will be an initial point of contact for the network in respect of any individuals with concurrent mental health and substance misuse issues. You will be required to link in with your counterpart within the Community network mental health teams.
A flexible and adaptable approach will be required.
A key part of the Band 6 role will include:
1. Providing recovery coordination of complex individuals presenting to TRI. This will include consideration of safeguarding needs, mental health comorbidity (Dual diagnosis) and multi-agency risk assessment conference MARAC.
2. Monitor patients who are receiving substitute prescribing interventions to support their recovery for opiate misuse or misuse of other substances.
3. Take responsibility for specific pathways and ensuring relationships and communication is clear between Torbay recovery initiatives and primary care networks and mental health services
4. Support and engage people who typically find it hard to access and utilise community services and resources.
5. Actively engage and work with individuals with a wide range of mental health problems, who may also have co-existing complex needs including homelessness, substance misuse, significant risk to self or others, are socially excluded, and who at times may present with extremely challenging behaviours.

6. A professional Nursing qualification, Social Work degree or equivalent, BSc/Diploma in OT
7. A willingness to complete a duel diagnosis training programme

8. In-depth knowledge of treatment of drug /alcohol dependence.

9. Experience in working within a community based multidisciplinary team working with people with substance misuse and concurrent mental health issues.
