Here at Bluebird Care Manchester North & Salford we are seeking to appoint a highly motivated, passionate, and dynamic Care Coordinator to join us in developing our home care service. Roles and Responsibilities as a Care Supervisor With your knowledge and experience of great customer care, you will be a role model for other Care Assistants. Care Coordinator duties include but are not limited to: Mentor and coach care assistants. Visiting customers to review their tailored care and support plan, making any changes required. Introducing Care Assistants to the customer, keeping the customer up to date with any changes. Liaising with GPs, Pharmacies & District Nurses to ensure a joined-up approach. Regularly reviewing care notes and communicating any concerns. Provide safe care to ensure customers are comfortable with their care. Create and review customer support plans and risk assessments.
