An exciting opportunity has arisen for an enthusiastic and motivated pharmacist to join the Pharmacy team at HMP & YOI Parc. This unique setting combines elements of Primary Care, Hospital and Community pharmacy. This role will provide you with the opportunity to develop a person-centred care pharmacist service whilst also enhancing your clinical experience in a unique setting.
The post holder will work within the Pharmacy team and will work closely with the primary care multi-disciplinary team, including GPs, nurses, and Psychiatrists to ensure safe and effective prescribing. This role involves working with patients with long term conditions, mental health problems, and substance misuse problems.
This is a challenging but rewarding role for an individual who is an excellent communicator, confident and able to exercise strong leadership and management skills to deliver a pharmacy service across Primary Care, Integrated Substance Misuse and Mental Health Services. The post holder will also be required to work alongside colleagues within the wider Cwm Taf Morgannwg Health Board in order to support optimisation of patient care, safe transfer and coordination of care, and development of pharmacy service delivery at HMP & YOI Parc.
Independent prescribing is desirable but is not an essential pre-requisite for this role. However, the successful applicant must be willing to undertake the independent prescribing qualification if not already achieved.
Main duties of the job
The successful applicant will work in practices in the North cluster of Bridgend locality. The role will involve provision of medicines optimisation support and advice to the designated GP practices in the locality. If you thrive on providing expert advice on medicines, addressing polypharmacy and complex clinical medication reviews closer to the patient's home environment, then this is the role for you.
In addition to using your clinical skills at the selected GP practices, there will be opportunities to contribute to the community MDTs. You should have excellent communication skills and the ability to work effectively with other Health Care Professionals and primary care staff.

The Medicines Management team will provide you with support to integrate into the practices and teams.
There will be opportunities for both full time and part time working. We are looking to recruit at an 8a level.

Salary: £54,550 to £61,412 a year per annum pro rata.
