Jessie May delivers community-based palliative nursing care to families caring for children and young people with life-limiting and life-threatening conditions across various locations. The areas that Jessie May covers include South Gloucestershire, Bristol, North Somerset, BANES, Swindon, and Wiltshire. The service delivery hours are 08:00-20:00, including weekends, bank holidays, and overnight on-call as part of the 24/7 end-of-life care provision.
The post holder will take delegated responsibility for a specific geographical area, leading and coordinating the day-to-day activities of the Care Team within this area. Occasionally, they will serve as the Duty Lead for the entire service, providing support to Jessie May nurses across all geographical regions. Additionally, the post holder will deliver skilled, holistic nursing care and guidance to children and young people with complex medical needs and support their families. They will also be responsible for planning, implementing, and evaluating individualized care to ensure the highest standards of care are met.
Main Duties of the Job
The post holder will possess excellent communication skills and demonstrate the ability to build and maintain strong, supportive relationships with the children and families on the caseload. They will work collaboratively and effectively with a wide range of stakeholders, including healthcare staff, primary care teams, social services, educational institutions, and other relevant professionals. Their role will be pivotal in ensuring a coordinated, multi-disciplinary approach to care, enhancing the wellbeing of the children and young people under their responsibility while ensuring seamless service delivery across the Jessie May caseload.

Knowledge and Experience
* Experience of taking the lead in supporting staff within their role
* Knowledge of the importance of family-centered care and the challenges faced by families on a palliative care pathway
* Relevant post-registration experience, including palliative care and/or end-of-life care
* Shows evidence of teamwork and the ability to communicate effectively with the multidisciplinary team
* Recognizes own limits and seeks appropriate support, including drawing on evidence-based practice and accessing individualized training as needed
* Knowledge of safeguarding and shows appropriate, timely action to minimize risk in line with local policies and procedures
* Experience of needs assessment/care planning for complex needs

Qualifications and Training
* RSCN/RN (Child)
* Educated to Diploma or Degree level or working towards this
* Community Children's Nursing qualification
