Responsibilities

4. Monitor client progress and adjust care plans as necessary, advocating for clients' needs.
5. Maintain accurate records of client interactions, services provided, and any changes in care requirements.
6. Assist clients in navigating healthcare systems, providing guidance on available resources and services.
7. Conduct regular assessments to evaluate the effectiveness of care plans and make recommendations for improvements.
